Delighted to see our Legs Get Together clinic, delivered in partnership with Guildford East Primary Care Network, recognised with a Chief Nursing Officer Silver Award.
The clinic brings together Procare community nursing, practice nurses and social prescribing to provide joined-up leg care in a community setting, while also creating a supportive space for patients to connect with others.
It’s a great example of what can be achieved when primary care and community teams work together around patients’ needs.
