Confidentiality is the duty of a person to not disclose anything learned from a patient who has attended, consulted or been treated, without that person’s consent. I understand that all information about patients held by Procare is strictly confidential and that this confidentiality is permanent. I also understand that the duty of confidentiality owed to a person under 16 is as great as the duty owed to any other person. I will not disclose personal information about any person learnt in the course of my work, including the fact that they may have visited NHS services, even after I have ceased working for Procare. If I consider disclosure is necessary in the public, patient or family interest, I will discuss this with the appropriate health professional. If I hold a professional qualification, and my right to practice depends on that qualification being registered with a governing body, it is my responsibility to have read and understood their advice on confidentiality. I understand that a breach of this obligation may result in my work being terminated and that other appropriate action such as legal action may be taken.

CONFIDENTIALITY AGREEMENT

I have read the above and agree to be bound by its terms.
