Working in partnership
We support a population of over 220,000 people across Guildford and Waverley, working with partners across the NHS and local system to deliver care that is joined-up, equitable, and rooted in neighbourhoods.
As a GP Federation, we play an active role in place-based working — collaborating with Surrey Heartlands Integrated Care System, Royal Surrey NHS Foundation Trust, the Guildford and Waverley Alliance, and others to help shape services that reflect local priorities and population health needs.
Whether delivering care directly or supporting practices to innovate and adapt, we’re committed to working together for the health and wellbeing of the communities we serve.


Royal Surrey Hospital
We work in partnership to deliver adult community nursing across Guildford and Waverley. This joint model brings together hospital, community and general practice expertise, helping to improve continuity of care, reduce hospital admissions, and support patients to remain safely at home

Surrey Heartlands ICB
We support the system-wide ambition to improve population health and reduce inequalities by delivering services in neighbourhoods, enabling proactive and preventative care, and working at scale with primary care providers.

Guildford and Waverley Alliance
A partnership of local NHS organisations, the voluntary sector and social care. Through this place-based Alliance, we help shape and deliver integrated care locally, working together to better meet the health and wellbeing needs of our population.

Phyllis Tuckwell Hospice Care
Providing a range of free services to ensure that adult patients and families living with an advanced or terminal illness can make the most of their lives.

Midhurst Pallative Care
Providing specialist palliative care support to patients and families affected by complex life limiting illness.
